Request forms
The UAB offers you several generic request forms to manage your request for changes in personal data, change of name, to request the refound of the amount paid in the registration or a general model for other requests that are not contemplated in the different procedures of the information of the web. We remind you that if you consult the procedure corresponding to the procedure that you want to carry out in the different sections of this academic information, you will find the related application, particularized for each procedure.
In the request you must indicate all your information, such as name and postal address to notify you what corresponds. And above all, don't forget to sign it! You can sign it by hand or with an electronic signature (see this link to learn more about the electronic signature).
You can submit the application through the UAB Electrònic Office
- Request – Change of personal data. When the registration or modification of personal data is made after the admission process or the first registration.
- Request – Change of name. In this link you can find the Protocol of this procedure.
- Request for reimbursement of public prices. To request a refund of the registration fee.
- Application - General model. To request other procedures that do not have a specific model.
